  If you would like to attend Camp Edwards, you may Register Online.  If you prefer, you may print, complete and mail or fax the appropriate registration form to us.  The registration forms are available as PDF documents and require Adobe's Acrobat Reader. If you do not have Adobe's Acrobat Reader, it is available free from Adobe's Website.

   REGISTRATION FORMS that you may download:

• Family Camp

   REQUIRED FORMS that must be returned.

• Health Form - this must be current within 12 months from the date of camp.

• Physician Form - this must be current within 24 months from the date of camp.

• Release / Waterboggan / Parentpack Acknowledgement Form - A separate release form must accompany each registration. (not required for Women's Fitness or Family Camps)

• Medication Record for our Medical Coordinator.  Must be returned before your camp session begins if your camper will be taking meds at camp.  Parent or guardian will sign form at camp when they meet with Medical Coordinator.  This should save time in the "nurse line".

• Letter to My Leader/Letter to My Child's Leader - A separate letter must accompany each registration. (not required for Women's Fitness or Family Camps)
